Job summary

Metropolitan Jewish Health System, Inc. is seeking a Physician Assistant to provide and coordinate primary clinical care for assigned Nursing Home Residents.

You will act as a clinical resource to participating providers and facilities, ensuring development of care planning and access to preventive care aligned with member goals. The role requires graduation from an accredited PA program, NYS PA license, and a DEA certificate, with 1–2 years of geriatric care experience preferred.

Qualifications

• Graduate of an accredited Physician Assistant program.
• NYS Physician Assistant License is required.
• DEA certificate required.
• 1–2 years clinical experience as PA or NP, including 1 year in geriatric care.
• Managed care experience a plus.
• Proficiency with Word/Excel and EMR/case management software.

Responsibilities

• Provide and coordinate primary clinical care for assigned Nursing Home Residents.
• Act as resource and point of contact for member/designated representative regarding care plans.
• Collaborate as a team member and clinical resource to providers and facilities.

MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantagemembers who reside in participating long term care facilities. You will insure development of care planning and access topreventive care services in accordance with member and Plan health outcome goals and objectives and you will act as clinical resource to the participating providers and facilities.

• Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents
• Acts as resource and contact for member/designated representative regarding all aspects of Advantage member care plan
• Acts as team member and clinical resource to participating providers/facilities
• Acts as resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process

• Graduate of an accredited Physician Assistant program required.
• NYS Physician Assistant License required
• DEA certificate required
• One to two years clinical experience in role of nurse practitioner or physician assistant required including 1 year in a geriatric specialty.
• Previous clinical experience in acute, subacute and/or long term care setting preferred.
• Prior experience in managed care a plus.
• Proficiency in computer skills, electronic medical record, case management software, word, excel required.
