Job summary

MJHS is seeking a Nurse Practitioner to provide primary care coordination for Elderplan members residing in assisted living and long term care facilities across Nassau and Suffolk Counties. You will collaborate with physicians to develop care plans and deliver preventive services while supporting home-based care as needed.

Qualifications

• Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required.
• One to two years clinical experience in role of nurse practitioner required.
• Minimum one year in geriatric specialty.
• Previous clinical RN experience in acute, subacute and/or long term care setting preferred.
• Prior experience in managed care a plus.
• NYS NP License required.
• DEA certificate required.

• You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives

• You will act as a resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process

This position will cover Elderplan members who are residents of assisted living and long term care facilities in Nassau and Suffolk Counties.

MJHS is a large not-for-profit health system in the Greater New York area. Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.
