Job summary

Metropolitan Jewish Health System, Inc. (MJHS) is seeking a Nurse Practitioner to coordinate primary care for Advantage members residing in long‑term care facilities and living at home.

You will collaborate with physicians to ensure preventive care and optimal outcomes within a not-for-profit health system in the Greater New York area. The role requires an NP with geriatric experience, NYS RN/NP license, and a master’s degree in nursing.

MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. We also offer Elderplan/HomeFirst: health plans for Medicare and dual-eligible individuals. Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, you will provide and coordinate primary care services to an assigned panel of Advantage members who reside in participating long term care facilities.

• You will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives


• Provide and/or coordinate primary clinical care services for assigned Nursing Home Residents


• You will act as a clinical resource to the participating providers and facilities


• You will act as a resource and contact for member/designated representative regarding all aspects of member individualized care plan and disease process

• Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required


• One to two years clinical experience in role of nurse practitioner required


• Minimum one year in geriatric specialty


• Previous clinical RN experience in acute, subacute and/or long term care setting preferred


• Prior experience in manage care a plus


• NYS RN/NP License, Registration, DEA certificates
