Qualifications

• Licensed to practice as a PA in the employed state (KY)
• Current Controlled Substance License required at start date
• Current DEA License required at start date
• NCCPA certification
• Interest in Physiatry/PM&R
• Orthopedic experience is a plus
• Physiatry experience is a plus
• Skilled Nursing experience is a plus

Responsibilities

• Collaborate with therapists, nurses and other healthcare professionals to develop rehabilitation plans.
• Conduct patient assessments including histories and physical exams to set rehab goals.
• Monitor progress and adjust treatment plans to optimize function.
• Participate in multidisciplinary meetings and educational activities.
• Document evaluations, treatment plans, progress notes and discharge summaries in EMR.
• Educate patients and families about rehab process, outcomes and post-discharge care

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

Qualifications:

• Actively Licensed to practice in the state of which employed
• Current Controlled Substance License (required upon start date)
