Chestnut Health in Kentucky is seeking a compassionate Physician Assistant to join our physiatry team, delivering patient‑centered rehabilitation care and improving function and independence for diverse patients. You will collaborate with therapists, nurses, and other clinicians to design and implement comprehensive treatment plans and monitor progress in EMR documentation.

Formación

• Active license to practice in the state of employed.
• Current DEA license required on start date.
• Current Controlled Substances license required on start date.
• NCCPA certification.
• Interest to work in Physiatry/PM&R.
• Orthopedic experience is a plus.
• PM&R experience is a plus.
• Nursing home or SNF experience is a plus.

Responsabilidades

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
• Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
• Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence.
• Participate in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records.
• Provide education and counseling to patients and families regarding the rehabilitation process and post-discharge care.

Essential Duties & Responsibilities:

• Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.

• Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence.
• Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills.
• Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.
• Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care.

Qualifications:

• Actively Licensed to practice in the state of which employed
• Current Controlled Substance License (required upon start date)
• Current DEA License (required upon start date)
• NCCPA Certificate
• Desire to work in Physiatry/Physical Medicine and Rehabilitation
• Orthopedic Experience (nice to have)
• Physiatry Experience (nice to have)
• Skilled Nursing Experience (nice to have)

License/Certification:

• Certified Physician Assistant (Required)
