Chestnut Health Company is seeking a Physician Assistant to join our Physiatry team. This role emphasizes collaboration with therapists, nurses, and other clinicians to restore function and improve quality of life in patients with rehab needs.

Qualifications

Active license to practice in the state of employment.
Current DEA license required at start date.
NER: NCCPA certification is required.
Desire to work in Physiatry/PM&R.

Responsibilities

Collaborate with therapists, nurses, and other professionals on rehab plans.
Conduct patient assessments, medical history, exams, and evaluations.
Monitor progress and adjust rehab plans for independence.
Participate in multidisciplinary meetings and Education activities.
Document evaluations, plans, progress, and discharge summaries in EMR.
Educate patients and families about rehab process and aftercare.

Essential Duties & Responsibilities:

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions
Monitor patient progress closely and adjust treatment plans as necessary to optimize functional outcomes and promote independence
Participate actively in multidisciplinary team meetings, case conferences, and educational activities to enhance clinical knowledge and skills
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems
Provide education and counseling to patients and their families regarding the rehabilitation process, expected outcomes, and post-discharge care
