Optimal Care - Evergreen is looking for a Physician in Maumee, Ohio. The Physician will provide primary and specialty medical care primarily to seriously ill patients in home and other settings.

Job description

Key Responsibilities

Physicians provide primary and/or specialty medical care in a variety of ambulatory, home and long‑term care settings. The Physician is responsible for providing medical leadership to advanced practice providers such as Nurse Practitioners and Physician Assistants. The Physician will function in collaboration with providers according to a Collaboration Arrangement. The Physician’s practice will focus on caring for the seriously ill population while providing advanced primary care capabilities to assist patients and families with comfort and quality of life.

In this role you will be responsible for:

  • Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.
  • Performing independently under stress and in emergency situations.
  • Driving between locations, navigating to each location using GPS device or other tools.
  • Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, family, facility and other personnel involved with patient care.
  • Assessing health status by obtaining relevant health and medical history. Performing a physical exam based upon the age and patient history. Identifying significant health and medical risk factors.
  • Ordering, interpreting and evaluating laboratory tests and other diagnostic studies to assess patient’s clinical problems and health care needs. Developing a diagnosis by analyzing and synthesizing collected data.
  • Performing diagnostic and therapeutic interventions consistent with certification, law and state medical privileges. May prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.
  • Developing a treatment plan in collaboration with the patient and significant others to include Medicare, CMS, Ancillary services, and treatment. Collaborating with nursing, medical and other healthcare team members to ensure quality patient care.
  • Referring patients to providers as appropriate for consultation or for specialized health resources and treatment. Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas as necessary.
  • Supervising nurse practitioners and physician assistants per the Collaborative Agreement.
Required Qualifications
  • Doctor of Medicine or Osteopathy required
  • Current unrestricted State license to practice medicine.
  • Requires demonstrated interpersonal, supervisory, and management skills with ability to communicate effectively
  • Demonstrates proven organizational development and project management
  • Exhibits working knowledge of regulations and reimbursement in home health care as well as proven leadership abilities
  • Can sit, stand, bend, lift, and move intermittently
  • Must be able to lift at least 50 lbs and bear the weight of an average adult effectively
  • Current and valid driver’s license
  • Reliable transportation and valid automobile insurance coverage
Desired Qualifications
  • Board certification in family practice, internal medicine, geriatric or palliative care preferred.
  • Preferred 2 years experience in healthcare, with a combination of acute care, geriatric and/or long‑term care practice.
  • Experience with geriatric patients preferred.
Location
  • Home Office: Maumee, OH
  • Main Service Area: Greater Toledo and surrounding areas
    • This position services patients in facilities and private home settings
Hours
  • 8:00 AM – 5:00 PM, Monday through Friday
Pay Range

$215,000 - $238,000 USD
