Optimal Care is seeking a physician to join our Physician Services team, delivering advanced primary and specialty care to geriatric and medically complex patients, often in patients' homes or care settings. You will lead and collaborate with Nurse Practitioners and Physician Assistants to provide thoughtful, patient-centered care that improves comfort and quality of life.

The role entails high travel within the Jackson, MI area with standard hours, backed by supportive education and back-office

Qualifications

• MD or DO required with current unrestricted state license to practice medicine.
• Board certification in family medicine, internal medicine, geriatric or palliative care.
• Experience with geriatric patients preferred.
• Reliable transportation and active driver's license and insurance.

Responsibilities

About the Role

Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Physician in Physician Services, you'll provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on delivering thoughtful, patient-centered care that improves comfort, function, and quality of life.

In this role, you'll conduct comprehensive medical evaluations, diagnose and manage acute and chronic conditions, develop individualized treatment plans, and coordinate care with patients, families, and interdisciplinary healthcare teams. You'll also provide medical leadership and collaborative oversight for Nurse Practitioners and Physician Assistants, helping ensure high-quality, coordinated care while mentoring fellow clinicians. Through your expertise and leadership, you'll play a vital role in improving patient outcomes, reducing unnecessary hospitalizations, and enhancing the quality of life for the patients and families you serve.

Location

• Home Office: Jackson, MI
• Main Service Area: Greater Lansing and surrounding areas
  • This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• 8:00 AM – 5:00 PM, Monday through Friday

Required Qualifications

• Doctor of Medicine or Osteopathy
• Current unrestricted State license to practice medicine
• Board certification in family practice, internal medicine, geriatric or palliative care
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Preferred Qualifications

• Minimum 2 years experience in healthcare, with a combination of acute care, geriatric and/or long-term care practice
• Experience with geriatric patients

Essential Skills and Competencies

• Demonstrated interpersonal, supervisory, and management skills with ability to communicate effectively
• Demonstrates proven organizational development and project management
• Exhibits working knowledge of regulations and reimbursement in home health care as well as proven leadership abilities

Pay Range

$215,000 - $238,000 USD
