Job summary

Optimal Care in Maumee, OH, seeks a Physician in Physician Services to provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to deliver thoughtful, patient-centered care.

This role offers flexibility, no weekends, holidays, or on-call requirements, and a limited patient census, supported by dedicated back-office teams and a network of physicians and APPs to sustain clinical excellence and value-based care.

Qualifications

  • MD/DO with unrestricted state medical license.
  • Board certification in family practice, internal medicine, geriatric or palliative care.
  • Prior supervisor/management experience.
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

  • Conduct comprehensive medical evaluations and diagnose/ manage acute and chronic conditions.
  • Develop individualized treatment plans and coordinate care with patients, families, and interdisciplinary teams.
  • Provide medical leadership and oversight for NPs and PAs; mentor fellow clinicians.
  • Help improve patient outcomes and reduce hospitalizations through coordinated care.

About the Role

Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Physician in Physician Services, you’ll provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on delivering thoughtful, patient-centered care that improves comfort, function, and quality of life.

This role is ideal for a physician who values flexibility, collaboration, and practicing medicine beyond the traditional clinic setting. Supported by dedicated back-office teams, streamlined administrative processes, and a collaborative network of physicians and advanced practice providers, you’ll be able to focus on clinical excellence while helping shape the future of value-based care.

In this role, you’ll conduct comprehensive medical evaluations, diagnose and manage acute and chronic conditions, develop individualized treatment plans, and coordinate care with patients, families, and interdisciplinary healthcare teams. You’ll also provide medical leadership and collaborative oversight for Nurse Practitioners and Physician Assistants, helping ensure high-quality, coordinated care while mentoring fellow clinicians. Through your expertise and leadership, you’ll play a vital role in improving patient outcomes, reducing unnecessary hospitalizations, and enhancing the quality of life for the patients and families you serve.

Location
  • Home Office: Maumee, OH
  • Main Service Area: Greater Toledo and surrounding areas
    • This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes
Hours
  • 8:00 AM – 5:00 PM, Monday through Friday
Required Qualifications
  • Doctor of Medicine or Osteopathy
  • Current unrestricted State license to practice medicine
  • Board certification in family practice, internal medicine, geriatric or palliative care
  • Prior supervisor / management experience
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage
Preferred Qualifications
  • Minimum 2 years experience in healthcare, with a combination of acute care, geriatric and/or long-term care practice
  • Experience with geriatric patients
Essential Skills and Competencies
  • Demonstrated interpersonal, supervisory, and management skills with ability to communicate effectively
  • Demonstrates proven organizational development and project management
  • Exhibits working knowledge of regulations and reimbursement in home health care as well as proven leadership abilities
Pay Range

$215,000—$238,000 USD
