Job summary

Humana Inc. invites qualified physicians to join Conviva Greenacres Primary Care in Greenacres, Florida. The role focuses on hands-on patient care in a senior-focused model, with longer visits and a holistic approach to health determinants.

You will work 8:00am-5:00pm, Monday through Friday, with an established patient panel and in-person duties onsite. Board-certified or eligible in Geriatric, Family, or Internal Medicine; Florida license and DEA required.

Qualifications

• Must hold an MD or DO degree and be board certified or board eligible in Geriatric, Family Medicine or Internal Medicine.
• Valid, unrestricted license in the state of employment (Florida).
• Current, unrestricted DEA license.
• TB screening may be required.

Responsibilities

• Provide hands-on primary care in a senior-focused model with extended visits.
• Manage a patient panel with 14–17 patients per day and in-person care.
• Work 8:00am–5:00pm, Monday–Friday, onsite at Conviva Greenacres.
• Address social determinants of health impacting patient outcomes.

Job Overview

Become a part of our caring community CenterWell / Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Greenacres, Florida. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a versatile approach. As a Primary Care Physician, you will report to our Associate Medical Director. You will work onsite and in-person at the Conviva Greenacres location. A day in the life of a CenterWell physician Monday-Friday 8:00am-5:00pm Join an experienced team with an established patient panel See 14-17 patients per day. Opportunity to spend more time with patients Longer authorized appointments (45 minutes) Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care.

Required Qualifications

• Doctor of Medicine or Osteopathic Medicine Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
• Valid, unrestricted license in the state of desired employment (or willing to obtain)
• Current, unrestricted DEA license
• If selected for this role, we will require you to undergo TB screening.

Preferred Qualifications

• Minimum of two+ years of experience
• 1+ years of experience managing Medicare Advantage panel of patients
• Knowledge of Medicare guidelines and coverage.
• Knowledge of HEDIS quality indicators.
• Spanish and English language fluency #LI-JR3

Language Proficiency Testing

Use your skills to make an impact Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Scheduled Weekly Hours & Pay Range

Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $255,000 - $298,500 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
