Job summary

Pinnacle Care Providers is seeking a physician to join our post-acute medical practice serving skilled nursing and senior living communities in northeast Ohio. The role travels across facilities in the region and emphasizes collaborative care with APPs, care coordinators, and facility teams.

The physician will perform onsite rounds, manage complex geriatric conditions, and participate in quality initiatives with a strong focus on transitions of care and patient-centered plans.

Qualifications

  • MD or DO with an active, unrestricted medical license in the applicable state.
  • Board certified or board eligible in an appropriate specialty and eligible to participate in Medicare and Medicaid.
  • Strong clinical judgment, communication skills, and ability to work effectively within an interdisciplinary team.
  • Experience in geriatrics, internal medicine, family medicine, skilled nursing, long-term care, or post-acute medicine preferred.

Responsibilities

  • Provide scheduled onsite physician rounds at assigned facilities and manage acute and chronic conditions for medically complex older adults.
  • Collaborate closely with Pinnacle Advanced Practice Providers and Care Coordinators to develop and oversee patient-centered plans of care.
  • Support transitions of care, hospitalization follow-up, goals-of-care discussions, medication management, and efforts to prevent avoidable emergency department visits and hospitalizations.
  • Participate in quality improvement, value-based care, population health, and care-gap closure initiatives.
  • Maintain timely, accurate, and compliant medical documentation and coding.
  • Build strong working relationships with facility Administrators, Directors of Nursing, and clinical teams.
  • Participate in facility quality meetings and clinical initiatives as appropriate and potentially serve as Medical Director for one or more skilled nursing facilities.

Job description

Pinnacle Care Providers is seeking a physician to join our growing post-acute medical practice serving skilled nursing, long-term care, assisted living, and senior living communities. Our physicians provide consistent onsite clinical care while working closely with Advanced Practice Providers, Care Coordinators, facility leadership, nursing teams, patients, and families.

This position will travel in north-east Ohio region so candidates may live anywhere in the region (Cleveland, Canton, Akron or surrounding areas).

Key Responsibilities
  • Provide scheduled onsite physician rounds at assigned facilities and manage acute and chronic conditions for medically complex older adults.
