Job summary

Lorian Health in San Jose, CA seeks a Home Health Occupational Therapist. You will provide skilled OT services per physician orders, assess patients' functional abilities, and contribute to the Plan of Care updates to support independence at home.

Qualifications

• OT degree from an accredited program, with state licensure as an OT.
• Current CPR certification and a valid driver's license are required.
• At least one year of recent experience in home health or hospice.

Responsibilities

• Provide skilled occupational therapy services in accordance with physician orders and the patient’s individualized Plan of Care (POC).
• Evaluate patients’ functional abilities and assist the physician and care team in determining appropriate treatment goals and interventions.
• Contribute to the development and ongoing updates of the patient’s Plan of Care based on functional needs and progress.
• Assess the home environment and recommend appropriate modifications, equipment, or strategies to promote safety, independence, and quality of life.
• Monitor patient progress and communicate relevant changes and recommendations with the physician, case manager, patient, and family/caregivers.
• Educate and collaborate with patients, families, and LORIAN team members regarding therapy programs, techniques, and strategies to support continued progress.
• Maintain accurate, timely clinical documentation in accordance with LORIAN policies and procedures, while participating in continuing education and in-service programs.

Location: San Jose Northeast an, Northwest and Southwest

Schedule

Availability: 3 weekdays and 1 weekend per month.

As a Home Health OT, You Will

Required Qualifications

• Graduate of an occupational therapy school accredited by the Council of Medical Education of the AMA in collaboration with the American Association of Occupational Therapists and/or by the State Department of Education in the specific state where employed.
• Registered by the Occupational Therapist Association and licensed in the state of practice if the state has registered practices.
• Valid driver’s license and insured personal vehicle (per Lorian requirements).
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possesses and maintains current CPR certification.

This position is based on a pay per visit compensation model. Pay per visit ranges from $50 to $140 visit commensurate with experience and depending on type of visit.
