Qualifications

• Bachelor's degree in Physical Therapy with five years of progressive experience.
• Master's degree requires two years of progressive experience.
• Doctorate degree satisfies basic requirement for practice.
• Full, current, unrestricted license to practice Physical Therapy.

Responsibilities

• Participates in admissions process and develops interventions to maximize function.
• Collaborates with interdisciplinary team for patient care planning.
• Liaises with staff to communicate concerns to the team.
• Manages outside consults and coordinates admissions from various sources.
• Documents appropriately in Electronic Medical Record.
• Develops and implements program compliance and performance improvement plans.
