Job summary

Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities. A supportive, mentorship-driven culture awaits, with opportunities for professional growth and impact on residents’ recovery and quality of life.

Responsibilities

Utilizes evidence-based practice to support clinical interventions.
Provides rehabilitative, skilled and medically necessary treatment interventions to patients under the supervision of a Physical Therapist.
Provides skilled therapy services to patients needing rehabilitation.
Assesses and documents patient progress and communicates with the supervising Physical Therapist to adjust treatments.
Provides consultation and counseling to patients, families, caregivers and other service providers.
Provides input for discharge summaries per regulatory and clinical requirements.
