Continuum Rehab Group seeks a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities. We offer a supportive culture focused on compassion, growth, and meaningful patient impact in residents' recovery and quality of life.

Licensed PTAs at all experience levels are welcome; you will work under supervision of a Physical Therapist, deliver skilled interventions, document progress, and collaborate with families and care teams to optimize patient

Qualifications

• Licensed Physical Therapy Assistant credential.
• Open to therapists at all experience levels.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress and communicates with supervising PT.
• Provides consultation to patients and families.
• Contributes to discharge summaries per regulatory and clinical requirements.
