Evolve Therapy Services, LLC is seeking a dedicated Physical Therapist Assistant to join our in-house Skilled Nursing Facility team in Durham, NC. The PTA will support the Physical Therapist by delivering individualized therapy services, conducting screenings, and contributing to care plans under supervision.

The position requires an active PTA license, an associate degree in physical therapy, and commitment to ongoing education.

Formación

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilidades

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluating resident progress and updating treatment plans with the Physical Therapist.
• Develop and modify individual resident treatment plans and schedules.
• Educate residents, families, and staff on therapies and home exercise programs.
