Job summary

Bridge Home Health & Hospice in California seeks a licensed Physical Therapy Assistant to provide PT services under the supervision of a licensed Physical Therapist and according to the physician's plan of care.

You will guide patients through therapeutic exercises, supervise gait training, and use modalities while documenting progress and participating in home care team meetings and inservice education.

Qualifications

• Graduate of an accredited PTA program with APTA-approved degree.
• Licensed as Physical Therapy Assistant in California.
• Current CPR certification.
• Valid California driver's license and reliable transportation.

Responsibilities

• Provide direct physical therapy under supervision and in accordance with the plan of care.
• Treat patients to relieve pain and restore function.
• Lead and assist patient in therapeutic exercises and gait training.
• Apply physical agents (heat, cold, ultrasound, etc.).
• Observe and report patient responses and changes in condition.
• Instruct patients in wheelchair, braces, crutches, canes, and device use.
• Maintain clinical records and notes in prescribed time frames.
• Participate in home care team meetings and staff education.
• Adhere to attendance and reliability guidelines.
