Job summary

Actify Home Care, Inc. seeks licensed PTA to join a clinician-owned home health team delivering geriatric care in North Broward County, including Pompano Beach. This rapid-onboarding opportunity offers per-visit compensation and fast credentialing to get you working quickly.

You will provide one-on-one therapy in patients’ homes, follow treatment plans, document progress, and collaborate closely with the supervising PT. Kinnser/WellSky familiarity is a plus.

Qualifications

• Active Florida PTA license required.
• CPR/BLS certification required.
• Valid driver’s license and reliable transportation.
• Kinnser/WellSky experience is a major plus!

Responsibilities

• Provide high-quality, one-on-one therapy to geriatric patients in their homes.
• Implement and follow treatment plans established by the supervising PT.
• Chart patient progress and treatment responses accurately and efficiently.
• Coordinate and communicate patient status regularly with the supervising PT.
