Actify Home Care is seeking a licensed PTA to deliver one-on-one geriatric therapy in patients' homes across South Palm Beach County. You will implement PT plans, document progress, and collaborate with the supervising PT to ensure high-quality care.

Qualificações

• Active Florida PTA license required.
• Current CPR/BLS Certification.
• Valid Driver’s License and reliable transportation.

Responsabilidades

• Provide high-quality, one-on-one therapy to geriatric patients in their homes.
• Implement and follow treatment plans established by the supervising PT.
• Chart patient progress and treatment responses accurately and efficiently.
• Coordinate and communicate patient status regularly with the supervising PT.

What You Need:

• License: Active Florida Physical Therapist Assistant (PTA) License.

• Certification: Current CPR/BLS Certification.
