ConnectedHealth in Salt Lake City is seeking a Physical Therapist Assistant to join our rehabilitation team in a full-time, permanent role. You will support a licensed Physical Therapist by delivering targeted therapies and promoting patient independence.
You will document progress, communicate outcomes with the therapy team, educate patients and families, and help implement individualized treatment plans while adhering to state regulations and facility policies.
Physical Therapist Assistant (PTA) is a full-time permanent role in Salt Lake City, Utah, supporting a rehabilitation team to deliver individualized patient care. Deliver physical therapy treatments under the direction of a licensed Physical Therapist
Assist patients in improving strength, mobility, balance, coordination, and functional independence
Implement individualized treatment plans designed by the supervising Physical Therapist
Monitor patient progress and communicate treatment outcomes to the therapy team
Accurately document patient care, progress notes, and treatment activities
Educate patients and family members regarding treatment plans, exercises, and injury prevention techniques
Collaborate with physicians, therapists, nurses, and other healthcare professionals to ensure quality patient care
Maintain compliance with facility policies, state regulations, and professional standards
Associate Degree from an accredited Physical Therapist Assistant program
Current Utah Physical Therapist Assistant license or eligibility to obtain licensure
Current CPR/BLS certification
Strong communication, interpersonal, and organizational skills
Ability to work effectively in a collaborative healthcare environment
Experience in outpatient, inpatient rehabilitation, skilled nursing, or orthopedic settings
Strong patient-centered care approach
Ability to adapt to a variety of patient populations and treatment plans
Excellent time-management and documentation skills
You hold an associate degree and Utah PTA license. You can deliver therapy under a PT's direction. You excel at documentation, communication, and patient education
