ConnectedHealth in Ogden, Utah is seeking a licensed Doctor of Physical Therapy to provide comprehensive patient care, develop individualized treatment plans, and apply evidence-based interventions.
You will collaborate with physicians and the healthcare team, educate patients and families, and document clinical progress while maintaining high standards of patient safety and professional ethics.
Physical Therapist provides comprehensive care to patients in Ogden, Utah, using assessments, individualized treatment plans, & evidence-based interventions. The role supports functional recovery, patient education, & collaboration across the healthcare team. Licensed PTs will contribute to high-quality, patient-centered care in a collaborative environment.
Evaluate patients through comprehensive physical therapy assessments and develop individualized treatment plans.
Provide skilled physical therapy services based on patient needs, diagnoses, functional limitations, & established goals.
Implement therapeutic exercises, manual therapy, neuromuscular re-education, functional training, & other appropriate treatment techniques.
Monitor patient progress and modify treatment plans as clinically appropriate.
Educate patients and families on exercises, mobility strategies, injury prevention, home exercise programs, & long-term wellness.
Maintain accurate, timely, & thorough clinical documentation in accordance with organizational & regulatory requirements.
Collaborate with physicians, therapists, support staff, & other members of the healthcare team to promote coordinated patient care.
Communicate effectively with patients & caregivers regarding treatment plans, progress, & expected outcomes.
Promote patient safety and follow applicable clinical, ethical, & professional standards.
Participate in a positive, patient-centered work environment focused on quality outcomes & exceptional service.
Maintain professional knowledge & clinical skills through continuing education & ongoing professional development.
Doctor of Physical Therapy degree from an accredited physical therapy program.
Relevant clinical education & hands-on experience through an accredited physical therapy program.
Current or eligible Physical Therapist license in the State of Utah.
Ability to maintain an active, unrestricted professional license throughout employment.
Current CPR/BLS certification, or ability to obtain certification as required by the employer.
Graduation from a CAPTE-accredited physical therapy program.
Additional certifications or specialty training relevant to the position.
