Job summary

Canyons Springs Post Acute in California is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a full-time basis. Under the supervision of a licensed Physical Therapist, you will deliver quality rehabilitation services to residents in a post-acute setting.

Responsibilities include implementing treatment plans, documenting progress, collaborating with the care team, educating residents and families, and assisting with equipment recommendations.

Qualifications

• Current California PTA license or applicable authorization to practice.

• Help residents improve strength, balance, mobility, coordination, and functional independence.
• Document treatments, resident progress, and services accurately and timely.
• Communicate with the supervising Physical Therapist regarding progress, goals, and discharge planning.
• Collaborate with nursing and rehabilitation staff to support resident outcomes.
• Educate residents and families on home exercise programs, fall prevention, safety techniques, and mobility strategies.
• Assist with training Restorative Aides and staff education as needed.
• Recommend durable medical equipment to promote safety and independence.
• Maintain a clean, safe treatment environment and properly maintain equipment.
• Participate in care conferences and rehabilitation program development.

Canyons Springs Post Acute is seeking a compassionate, motivated, and dedicated Physical Therapist Assistant (PTA) to join our rehabilitation team! If you are passionate about helping residents regain strength, improve mobility, and achieve greater independence, we would love to hear from you.

As a PTA, you will work under the direction of a licensed Physical Therapist to provide quality rehabilitation services and personalized resident care in a supportive post-acute healthcare setting.

What You'll Do

• Provide physical therapy treatments according to established plans of care and the supervising Physical Therapist's direction.
• Help residents improve strength, balance, mobility, coordination, and functional independence.
• Document treatments, resident progress, and services accurately and in a timely manner.
• Communicate with the supervising Physical Therapist regarding progress, treatment goals, and discharge planning.
• Collaborate with nursing, rehabilitation, and interdisciplinary team members to support resident outcomes.
• Educate residents, families, and caregivers on home exercise programs, fall prevention, safety techniques, and mobility strategies.
• Assist with training Restorative Aides and provide staff education as needed.
• Recommend appropriate durable medical equipment, such as walkers, canes, splints, and other mobility aids, to promote resident safety and independence.
• Maintain a clean, safe treatment environment and ensure equipment is properly maintained.
• Participate in care conferences, quality improvement initiatives, and rehabilitation program development as assigned.
• Maintain resident confidentiality and comply with applicable state and federal regulations, facility policies, and infection control standards.
