Job summary

Woodlands Health and Rehabilitation Center is seeking a dedicated Physical Therapy Assistant to join our skilled nursing facility team. You will help residents recover function and achieve therapy goals through individualized treatment plans in a supportive setting.

Responsibilities include guiding patients in exercise programs, educating on post-discharge maintenance, and collaborating with the therapy team to ensure high-quality care. Strong communication and computer skills are essential.

Qualifications

• Must have an Associate’s Degree in a Physical Therapy Assistant program from an accredited college or university.
• Must possess a current, unencumbered PTA license in Ohio to practice.
• Must possess basic computer skills and ability to navigate through software and EHR.

Responsibilities

• Return residents to optimal physical function in SNF settings.
• Instruct residents in maintaining function through education and independent exercise.
• Support patient, physician, and therapy goals and participate in discharge planning.

Position Summary:

The primary purpose of your job is to return resident to optimal physical function and facilitate achievement of patient, physician, and therapy goals in a SNF setting. Instruct the patient in maintaining achieved level of function after discharge through education, independent exercise program, and appropriate activity modification.

Education/Qualifications:

• Must have an Associate’s Degree in a Physical Therapy Assistant program from an accredited college or university.
• Must possess a current, unencumbered license to practice as a Physical Therapy Assistant within the state in which you work.
• Must possess basic computer skills and ability to navigate through various software programs, including service delivery software and the electronic health record software.
