Job summary

Lifepoint Rehabilitation in Northlake, IL is seeking a part-time Physical Therapist Assistant (PTA) to support therapy services and patient care. You will implement treatment plans under supervision, monitor progress, document outcomes, and assist with discharge planning.

Responsibilities

• Implements treatment plan developed by supervising therapist using appropriate modalities and seeks consultation as necessary.
• Monitors patients’ responses to the treatment plan and regularly communicates patient progress and possible goal revisions to the physical therapist.
• Documents services provided and patient response/progress.
• Educates the patient and family/caregiver about patient deficits and assists with patient discharge planning.
• Provides information regarding appropriate selection and use of adaptive equipment and community support programs.
