Job summary

Lifepoint Rehabilitation is seeking a Physical Therapist Assistant (PTA) to join our compassionate team in Northlake, IL. You will support licensed therapists and help patients regain independence through supervised plans.

Responsibilities include implementing treatment plans, monitoring progress, documenting outcomes, educating patients and families, and assisting with discharge planning while coordinating with the supervising therapist to ensure high-quality care.

Qualifications

• Current state licensure as a Physical Therapy Assistant.
• Previous experience in the care of patients preferred.
• Current BLS/CPR.

• Monitors patient's responses to the treatment plan. Regularly communicates patient progress and possible goal revisions to the physical therapist.
• Documents services provided and patient response/progress.

• Provides information regarding appropriate selection/use of adaptive equipment and community support programs.

How you’ll contribute

A Physical Therapist Assistant who excels in this role:
