Job summary

Step Up Rehab is the in-house therapy provider at Madison Health and Rehabilitation Center. This PRN position serves residents in a Skilled Nursing Facility, offering skilled PTA services in collaboration with the interdisciplinary team to improve safety and quality of life.

Qualifications include graduation from an accredited PTA program and an active Florida PTA license (or license-eligible); SNF or post-acute experience is preferred, with Net Health documentation knowledge being a plus.

Work inside the dedicated therapy gym within this skilled nursing facility, helping residents regain function and independence through skilled, compassionate care.

Physical Therapist Assistant (PTA) – Skilled Nursing Facility (SNF)

PRN POSITION **Increased rates when medical benefits are waived.**

This position is exclusively for professionals licensed in the state of Florida as a SLP. All services are provided in a Skilled Nursing Facility (SNF).
