Job summary

Interim HealthCare in Deltona, FL is seeking a Home Health Physical Therapist Assistant (PTA) to join our PRN team. You’ll provide home-based therapy to patients, helping them regain mobility and function in familiar surroundings.

Work with a coordinated home health team, document progress, adjust plans as needed, and educate patients and families on exercises, safety and self-care. This role offers flexible scheduling, market-competitive visit rates, mileage reimbursement and an EMR that

Qualifications

• Minimum of 1 year of physical therapy assistant experience, ideally in home healthcare.
• CPR certification is required and must be maintained.
• Knowledge of state and federal home health regulations is preferred.

Responsibilities

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury.
• Work as part of a home health team to implement the patient’s plan of care and goals.
• Observe movements, document progress and modify care plans as needed.
• Assist patient with exercises and hands‑on therapy to manage pain and increase mobility.
• Educate patients on their plan of care, established goals, exercises and self-care.
• Coach family on the plan of care, therapy goals and safely assisting with exercises.

As a Home Health Physical Therapist Assistant, here’s a big-picture view of whatyou’ll do:

• Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient’s plan of care and goals
• Observe movements, conduct assessments, document progress and modify care plan as needed

• Educate patients on their plan of care, established goals, exercises and self-care
• Coach family on the plan of care, therapy goals and safely assisting with exercises

A few must-haves for Home Health Physical Therapist Assistants:

• Graduate of an accredited Physical Therapist Program and active PTA license in Florida
• Minimum of 1years of physical therapy assistant experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, strong interpersonal skills, dexterity and compassion
