Job summary

Interim HealthCare of NE Florida is seeking a Home Health Physical Therapist in Deltona, FL and surrounding areas. Provide one-on-one home-based therapy to help patients regain strength, reduce pain and improve mobility in their daily environments.

You’ll operate within a supportive team that emphasizes autonomy and work-life balance. Requirements include graduation from an accredited Physical Therapist program and Florida PT license, minimum 1 year of PT experience (preferably in home health),

Qualifications

• Graduation from an accredited Physical Therapist program with Florida licensure.
• Minimum of 1 year of physical therapy experience, preferably in home healthcare.
• CPR certification and knowledge of state and federal home health regulations.
• Strong clinical judgement, interpersonal skills, dexterity and compassion.

Responsibilities

• Provide home-based physical therapy to patients to regain mobility, strength and functionality.
• Work as part of a home health team focused on the patient’s plan of care and goals.
• Observe movements, assess progress and modify care plan as needed.
• Educate patients on their plan of care, goals, exercises and self-care; coach family.
• Document progress and ensure safety in the home setting.
