Job summary

Coastal Home Health & Hospice is seeking a licensed Physical Therapy Assistant to provide in-home physical therapy services under physician orders and supervision of a Physical Therapist. The role focuses on improving function for home health and hospice patients in a rural setting.

The PTA will implement the plan of care, educate patients and families, and collaborate with the healthcare team to optimize patient outcomes and quality of life.

Qualifications

  • Graduate from a two-year college PTA program.
  • Licensed as a Physical Therapy Assistant in Oregon.
  • Current/Valid Driver’s License.

Responsibilities

  • Understands and adheres to Agency policies and procedures.
  • Provides physician-prescribed physical therapy under a PT plan of care.
  • Improves or minimizes residual physical disabilities.
  • Returns the patient to optimum and productive level.
  • Participates with other health care personnel in care planning.
  • Performs all skilled procedures as ordered by physician and per PT plan of care.
  • Consults with PT regarding treatment changes.
  • Instructs patients and family in home programs and daily living activities.
  • Participates in in-service programs and presents when assigned.
  • Participates in performance improvement activities as assigned.
  • Attends patient care conferences as scheduled.
  • Prepares medical records and progress notes and updates care plans.
  • Performs services planned and supervised by the PT.
  • Assists in preparing clinical and progress notes.
  • Educates the patient and family.
  • Other duties as assigned.

POSITION SUMMARY:

Performs physical therapy services for patients requiring Home Health or Hospice care, under the care of a physician and under the supervision of a Physical Therapist (PT), according to accepted standards of practice.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

May include, but are not limited to the following:

  • Understands and adheres to established Agency policies and procedures.

  • Provides physician prescribed physical therapy under a plan of care established by the PT.

  • Improves or minimizes residual physical disabilities of the patient.

  • Returns the individual to optimum and productive level within the patient’s capabilities.

  • Participates with all other health care personnel in patient care planning.

  • Performs all skilled procedures as ordered by physician and according to the plan of care established by the PT.

  • Consults with PT regarding change in treatment.

  • Instructs patients and family members in home programs and activities of daily living.

  • Participates in in-service programs and presents in-service programs as assigned.

  • Participates in performance improvement activities as assigned.

  • Attends all patient care conferences as scheduled.

  • Prepares medical records, progress notes and updates care plans for each patient visit in a timely manner as per Agency policy.

  • Performs services planned, delegated and supervised by the PT.

  • Assists in preparing clinical and progress notes.

  • Participates in educating the patient and family.

  • Other duties as assigned.

EDUCATION/EXPERIENCE/QUALIFICATIONS/REQUIREMENTS
  • Graduate from a two-year college level program approved by the American Physical Therapy Association.

  • Licensed as a Physical Therapy Assistant by the Oregon Physical Therapy Licensing Board.

  • Current/Valid Driver’s License.

PHYSICAL DEMANDS/REQUIREMENTS:
  • Regularly required to use hands and arms for reaching, handling, gripping, feeling and/ or massaging.

  • Ability to verbally communicate effectively.

  • Visual/hearing ability sufficient to comprehend written/verbal communication.

  • Ability to perform tasks involving physical activity, which may include heavy lifting of supplies (up to 50 pounds), equipment or patients; extensive bending, stooping, standing, walking or sitting may be required.

  • Ability to drive in a variety of adverse weather conditions, occasionally for lengthy distances or at night.

  • Ability to deal effectively with stress.

  • No known communicable diseases.

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

WORK CONDITIONS/ENVIRONMENT:
  • The noise level in the work environment is usually moderate.

  • Employee may encounter various environmental situations in individual patient homes, including odors, tobacco smoke or animals.

  • Working hours are generally 8:00 a.m. to 5:00 p.m. Monday through Friday.
