Job summary

Physicians Choice is seeking a compassionate Physical Therapy Assistant to join our home health team serving the Portland, Oregon area. You will deliver skilled PTA services in patients’ residences under the supervision of a licensed Physical Therapist, focusing on strength, mobility, independence, and safety.

You will educate patients and caregivers, document thoroughly, and collaborate with nurses, PTs, and other clinicians to ensure continuity of care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current and unrestricted Oregon PTA license or eligibility for Oregon licensure.
• Current CPR/BLS certification.
• Valid driver’s license, reliable transportation, and automobile insurance for travel to patient homes.
• Ability to safely perform the physical requirements of home health therapy.
• Strong communication, organizational, and time-management skills.
• Ability to work independently while coordinating with an interdisciplinary team.
• Comfortable traveling throughout the Portland metropolitan area and surrounding service territory.
• Knowledge of fall prevention, home safety, gait training, and functional mobility (preferred but required).
• Experience with electronic medical records and point-of-care documentation (preferred).

Responsibilities

• Provide skilled PTA interventions according to the Plan of Care under the supervising Physical Therapist.
• Implement therapeutic exercises, gait training, transfer training, balance activities, and mobility training.
• Support patients to improve functional mobility, strength, endurance, and independence.
• Educate patients, families, and caregivers on exercises, mobility, fall prevention, devices, and home safety.
• Monitor patient response and report significant changes to the PT and care team.
• Reinforce home exercise programs and provide ongoing education between visits.
• Identify safety concerns and barriers in the home environment.
• Coordinate care with the interdisciplinary team and communicate effectively.
• Complete accurate clinical documentation per policies and regulations.
• Maintain appropriate visit frequency and follow physician orders for visits.
• Assist with discharge planning by communicating progress toward therapy goals.
• Follow HIPAA and infection-control policies and procedures.

About the Role

Physicians Choice is seeking a compassionate, dependable, and motivated Physical Therapy Assistant (PTA) to join our home health team supporting patients in the Portland, Oregon area. In this full-time, home-based role, you will deliver skilled physical therapy services in patients’ residences under the direction and supervision of a licensed Physical Therapist. Success in this position means helping patients improve strength, mobility, independence, and safety—while providing patient-centered care, communicating clearly with the interdisciplinary team, and maintaining accurate, timely documentation to support continuity of care.

Duties & Responsibilities

• Provide skilled physical therapy interventions according to the established Plan of Care and under the direction of the supervising Physical Therapist.
• Implement therapeutic exercises, gait training, transfer training, balance activities, strengthening, mobility training, and other interventions ordered by the Physical Therapist.
• Support patients in improving functional mobility, strength, endurance, balance, and independence with activities of daily living.
• Educate patients, family members, and caregivers on therapeutic exercises, mobility techniques, fall prevention, assistive device use, and home safety.
• Monitor patient response to treatment and promptly communicate significant changes in condition, functional status, or progress to the supervising Physical Therapist and care team.
• Reinforce home exercise programs and provide ongoing patient and caregiver education between visits as appropriate.
• Identify and report potential safety concerns, changes in the home environment, and barriers that may affect progress toward therapy goals.
• Coordinate care and communicate effectively with the interdisciplinary team, including Physical Therapists, nurses, occupational therapists, speech-language pathologists, and home health aides.
• Complete accurate, thorough, and timely clinical documentation in accordance with agency policies, payer requirements, and applicable state and federal regulations.
• Maintain appropriate visit frequency, follow physician/provider orders, and communicate scheduling or missed-visit concerns to the appropriate team members.
• Assist with discharge planning by communicating patient progress toward established therapy goals.
• Follow HIPAA, infection-control, patient-safety, and emergency-preparedness policies and procedures.

Requirements

• Current and unrestricted Oregon Physical Therapist Assistant license or eligibility for Oregon licensure.
