Job summary

Visiting Nurse Association of SEMO seeks a dedicated Physical Therapist Assistant to provide skilled PT services in patients’ homes under physician orders.

Must hold a Missouri PTA license, have reliable transportation, and pass background checks. Minimum 1 year of experience preferred, with duties including scheduling, documentation, care coordination, and PPE compliance.

Qualifications

• Must be at least 18 years old.
• Must hold a current Missouri PTA License.
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment drug screening, criminal background check, registered w/Family Care Reg. and not be listed on the EDL.
• Must complete an application in full and furnish at least 2 credible business references with correct addresses and telephone numbers.
• Minimum of 1 year experience preferred.

Responsibilities

• Develop a schedule with the PT and patient that meets patient needs and physician orders.
• Provide direct therapy services to patients in accordance with the plan and assess patient conditions.
• Record patient responses to therapy and report to the PT.
• Maintain documentation and submit visit notes daily; document care coordination.
• Participate in patient care conferences.
• Maintain an adequate inventory of supplies and ensure equipment is cleaned and maintained.
• Make referrals for other disciplines as needed.
• Plan for necessary supplies and monitor costs; minimize waste.
• Wear all required PPE.
• All services provided per physician orders.

INTRODUCTION:

The Home Health program provides services for those needing skilled services under orders from a private physician delivered in the patient's private home. Payment is through Medicaid, Medicare and Private Pay/Insurance. Visiting Nurse Association of Southeast Missouri is a not for private organization.

Type Of Work Involved

• Works with the PT and TCM and the patient to develop a schedule that meets the needs of the patient, the orders of the physician, and VNA. The patient is to be informed in advance if the schedule is to be changed.
• Works with Physical Therapist to develop a plan of treatment and provide direct services to patients in accordance with the plan, including assessment of patients conditions.
• Services are to be provided in accordance with the physician's orders.
• Records and reports to the PT the patient's response to therapy.
• Maintains appropriate documentation and submits visit notes daily. Documents care coordination.
• Participates in patient care conferences.
• Maintains an adequate (non-consumable) inventory of supplies for use in the provision of therapy services.
• Responsible for the cleaning and maintenance of all equipment used.
• Makes referrals for other disciplines as needed.
• Plans for necessary supplies and maintains awareness of cost and unnecessary waste.
• Wears all Personal Protective Equipment as required.

TRAINING

Training involves a 30 day period where policies and procedures are covered. Computer training will cover the use of the hand held used to make patient visits.

Qualifications

• Must be at least 18 years of age
• Able to read, write, perform simple mathematics and follow directions
• Must hold a current Missouri PTA License
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment drug screening, criminal background check, registered w/Family Care Reg. and not be listed on the EDL
• Must complete an application in full and furnish at least 2 credible business references with correct addresses and telephone numbers
• Must be able to lift a minimum of 50# and sit for long periods of time
