Job summary

Visiting Nurse Association of Southeast Missouri is seeking a skilled physical therapy assistant to provide in-home physical therapy services in Cape Girardeau, MO. The role requires a Missouri PTA license and driving to patient homes, with documentation of visits and patient progress.

Responsibilities include developing treatment plans with the supervising PT, delivering direct care, coordinating with care teams, and maintaining equipment and supplies.

Qualifications

• Must be at least 18 years old.
• Able to read, write, perform simple mathematics and follow directions.
• Must hold a current Missouri PTA License.
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment drug screening and criminal background check.
• Must furnish at least 2 credible business references with addresses and phone numbers.
• Must be able to lift a minimum of 50 lb and sit for long periods.
• Minimum of 1 year experience preferred.

Responsibilities

• Develop a schedule with PT and patient needs, and inform patient of changes.
• Develop plan of treatment with the PT and provide direct services.
• Provide services per physician orders and document patient responses.
• Submit daily visit notes and care coordination records.
• Participate in patient care conferences and maintain equipment inventory.
• Ensure proper cleaning/maintenance of equipment and refer to other disciplines as needed.
• Plan supplies and monitor costs to avoid waste; wear PPE as required.

Job description

POSITION PURPOSE

Provide skilled physical therapy services to patients/clients.

INTRODUCTION: The Home Health program provides services for those needing skilled services under orders from a private physician delivered in the patient's private home. Payment is through Medicaid, Medicare and Private Pay/Insurance. Visiting Nurse Association of Southeast Missouri is a not for private organization.

TYPE OF WORK INVOLVED:

• Works with the PT and TCM and the patient to develop a schedule that meets the needs of the patient, the orders of the physician, and VNA. The patient is to be informed in advance if the schedule is to be changed.
• Works with Physical Therapist to develop a plan of treatment and provide direct services to patients in accordance with the plan, including assessment of patients conditions.
• Services are to be provided in accordance with the physician's orders.
• Records and reports to the PT the patient's response to therapy.
• Maintains appropriate documentation and submits visit notes daily. Documents care coordination.
• Participates in patient care conferences.
• Maintains an adequate (non-consumable) inventory of supplies for use in the provision of therapy services.
• Responsible for the cleaning and maintenance of all equipment used.
• Makes referrals for other disciplines as needed.
• Plans for necessary supplies and maintains awareness of cost and unnecessary waste.
• Wears all Personal Protective Equipment as required.

TRAINING:

Training involves a 30 day period where policies and procedures are covered. Computer training will cover the use of the hand held used to make patient visits.

Qualifications:

• Must be at least 18 years of age
• Able to read, write, perform simple mathematics and follow directions
• Must hold a current Missouri PTA License
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment drug screening, criminal background check, registered w/Family Care Reg. and not be listed on the EDL
• Must complete an application in full and furnish at least 2 credible business references with correct addresses and telephone numbers
• Must be able to lift a minimum of 50# and sit for long periods of time
• Minimum of 1 year experience preferred
