Job summary

Visiting Nurse Association of Southeast Missouri is seeking a licensed Physical Therapy Assistant to provide skilled physical therapy in patients' private homes under physician orders. The role supports the Home Health program serving Medicaid, Medicare and private pay clients.

Qualifications

• Must hold a current Missouri PTA License.
• Must be at least 18 years old.
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment drug screening and criminal background check.
• Must be registered with Family Care Reg. and not be listed on the EDL.
• Minimum of 1 year experience preferred.
• Must be able to lift a minimum of 50 pounds.
• Must complete an application in full and furnish at least 2 credible references with correct addresses and telephone numbers.
• Must read, write and perform basic mathematics.

Responsibilities

• Develop a schedule with the PT and patient to meet needs.
• Provide direct therapy per the plan of treatment.
• Document patient response and maintain records.
• Submit daily visit notes and care coordination.
• Participate in patient care conferences.
• Maintain inventory and clean equipment.
• Make referrals to other disciplines as needed.
• Plan for supplies and monitor costs.

Job description

POSITION PURPOSE

Provide skilled physical therapy services to patients/clients.

INTRODUCTION

INTRODUCTION: The Home Health program provides services for those needing skilled services under orders from a private physician delivered in the patient's private home. Payment is through Medicaid, Medicare and Private Pay/Insurance. Visiting Nurse Association of Southeast Missouri is a not for private organization.

TYPE OF WORK INVOLVED

• Works with the PT and TCM and the patient to develop a schedule that meets the needs of the patient, the orders of the physician, and VNA. The patient is to be informed in advance if the schedule is to be changed.
• Works with Physical Therapist to develop a plan of treatment and provide direct services to patients in accordance with the plan, including assessment of patients conditions.
• Services are to be provided in accordance with the physician's orders.
• Records and reports to the PT the patient's response to therapy.
• Maintains appropriate documentation and submits visit notes daily. Documents care coordination.
• Participates in patient care conferences.
• Maintains an adequate (non-consumable) inventory of supplies for use in the provision of therapy services.
• Responsible for the cleaning and maintenance of all equipment used.
• Makes referrals for other disciplines as needed.
• Plans for necessary supplies and maintains awareness of cost and unnecessary waste.
• Wears all Personal Protective Equipment as required.

TRAINING

Training involves a 30 day period where policies and procedures are covered. Computer training will cover the use of the hand held used to make patient visits.

QUALIFICATIONS

• Must be at least 18 years of age
• Able to read, write, perform simple mathematics and follow directions
• Must hold a current Missouri PTA License
• Must have reliable transportation and proof of automobile insurance.
• Must pass pre-employment drug screening, criminal background check, registered w/Family Care Reg. and not be listed on the EDL
