Job summary

Meadowood Nursing Center in Clearlake, CA is seeking a Licensed Physical Therapy Assistant to join a close-knit in-house therapy team. The role supports physical therapy services, documents treatments, and collaborates with an interdisciplinary team to enhance patient outcomes.

We welcome therapists at all experience levels. This position offers a flexible mix of full-time, part-time and PRN opportunities with benefits including health insurance, PTO, and retirement benefits.

Qualifications

• Licensed Physical Therapy Assistant.

• Strong communication and writing skills for documentation.

Responsibilities

• Treat patients as directed by the Physical Therapist and adhere to facility documentation guidelines.
• Record treatments given and progress notes in medical record.
• Assist in maintaining department.
• Participate in Patient Care and Rehabilitation Conferences.
• Assist with cleaning and maintenance of treatment area.
• Communicate with supervisor and other health team members regarding patient progress, problems and plans.
• Participate in training programs for other staff in the facility.
• Record treatment charges per the facility policy and procedures.
• Instruct patient' s families or nursing staff in maintenance program.
