REHAB STRATEGIES is seeking a dedicated Physical Therapist Assistant to join our care team. You will work with licensed physical therapists to deliver high-quality rehabilitation across inpatient, outpatient, and home health settings.

The ideal candidate has an associate degree in PTA, knowledge of anatomy and physiology, and strong communication skills to educate patients and families and participate in discharge planning.

Qualifications

• Associate degree in Physical Therapy Assisting is required.
• Knowledge of anatomy and physiology is essential.
• Experience in skilled nursing facilities or acute care settings preferred.
• Strong communication skills to interact with patients and healthcare teams.
• Ability to develop individualized care plans based on patient assessments.

Responsibilities

• Assist physical therapists in implementing treatment plans for patients with various conditions.
• Provide hands-on patient care using techniques such as trigger point therapy and physiotherapy.
• Monitor patient progress and adjust treatment plans under supervision of a licensed therapist.
• Educate patients and families on exercises and rehabilitation techniques to promote recovery.
• Maintain accurate documentation of patient treatments, progress notes, and care plans.
• Collaborate with healthcare professionals to ensure comprehensive patient care across settings such as home health, outpatient clinics, and SNFs.
• Participate in discharge planning to facilitate smooth transitions for patients leaving acute care or rehabilitation settings.
