Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Trotwood, OH. This PRN role supports the therapy team by delivering PT services under supervision, performing screenings, and helping implement individual treatment plans.

The ideal candidate holds an associate degree, maintains an active PTA license in the state, and commits to ongoing professional development and compliant CMS documentation.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services under the supervision of a Physical Therapist.
• Screen residents regularly to determine need for intervention.
• Assist in evaluations and treatment planning with the PT.
• Develop and modify individual treatment plans with goal setting.
• Conduct individual and group therapy sessions per plan.
• Monitor resident response and adjust interventions as needed.
• Document compliant and medically necessary therapy notes.
• Educate residents, families and staff on therapy activities.
