Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Dunn Loring, VA. The role is offered as Full Time, Part Time, or PRN, with responsibilities including direct PT supervision, screenings, goal-driven treatment plans, and resident education.

candidates must hold an Associate degree in PTA and an active PTA license, with ongoing continuing education to maintain credentials.

Qualifications

• Possession of an eligible Physical Therapy Assistant license in the practice state.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the PT in evaluations and in identifying targets, goals, and treatment plans.
• Conduct individual and group treatment focusing on strength, mobility, gait, ROM and modalities.
• Assist in selecting PT techniques/media and adapt per resident performance after PT consultation.
• Monitor resident response and coordinate with PT for any modifications.
• Assist in scheduling with PT and other team members to ensure treatment continuity.
• Adhere to resident schedules and modify as needed for treatment regimen.
• Educate residents, families and staff in use of equipment and therapy principles.
• Document therapy sessions in CMS-compliant NetHealth EMR systems and maintain records.
• Support Quality Assurance activities as required.
