Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Dyersburg, TN. You will work under the supervision of a Physical Therapist to deliver resident-centered therapy services.

Responsibilities include screenings, treatment planning, and hands-on therapy across mobility, strength, and gait retraining, plus documenting CMS-compliant records in NetHealth EMR.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Provide physical therapy services to residents under supervision.
• Conduct screenings to determine intervention needs.
• Assist in evaluations and in creating treatment plans with the Physical Therapist.
• Deliver individual and group therapy focusing on strength, mobility, gait, and balance.
• Document CMS-compliant notes in NetHealth EMR and coordinate with the care team.
