Job summary

The Pennant Group is seeking a licensed Physical Therapist Assistant to provide PT services under the direction of the PT and the physician’s plan of care in home health settings.

Responsibilities include patient treatment to relieve pain, restore or improve function, and support safe transfers and exercises, with documentation and teamwork across the organization.

Qualifications

• PTA must be licensed where the HHA is located if licensure is required.
• Graduate from a CAPTE-approved PTA program and pass a national PTA exam.
• Maintain current CPR certification.
• Must have and maintain a valid driver’s license and access to transportation for agency travel.
• Maintain automobile insurance and have access to a vehicle.

Responsibilities

• Provide direct physical therapy per the supervising PT and physician’s care plan.
• Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
• Assist patients with active and passive therapeutic exercises, gait and functional training, transfers, and prosthetic training.
• Apply physical agents including heat, cold, water, light, electricity, ultrasound, and massage.
• Observe and report patient reactions to treatment and changes in condition to the PT and organization staff.
• Instruct patients in use of wheelchairs, braces, crutches, canes, and orthotic devices.
• Maintain clinical notes and records within prescribed timeframes.
• Participate in home care team meetings and in-service education programs.
• Performs other duties as assigned.

Job Summary

The physical therapist assistant provides physical therapy services under the direction of the physical therapist and according to the physician's plan of care.

Duties & Responsibilities

• Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.
• Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
• Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
• Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.
• Participates in Organization Home Care Team Members meetings and Organization in-service Education Program.
• Performs other duties as assigned.

Job Requirements (Education, Experience, Knowledge, Skills & Abilities)

• A PT assistant must be licensed where the HHA is located if licensure is required.

• Possesses and maintains current CPR certification.
• The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
• Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.
