The Pennant Group is seeking a Licensed Physical Therapist Assistant to provide direct PT services in a home health setting in Colorado Springs, CO. You will work under the direction of a licensed Physical Therapist and follow each patient’s plan of care to improve function and relieve pain.

You will guide patients through therapeutic exercises, gait training, and device instruction, while documenting progress and coordinating with the home health team.

Formación

• Licensed PTA (CAPTE-approved) is required.
• CPR certification is required.
• Valid driver’s license and access to reliable transportation.

Responsabilidades

• Provide direct physical therapy under the direction of the PT and per plan of care.
• Treat patients to relieve pain and restore function.
• Guide patients through therapeutic exercises and gait/functional training.
• Utilize modalities such as heat, cold, ultrasound, massage, as directed.
• Monitor patient response and report changes to the PT and organization staff.
• Instruct patients on use of mobility aids and prosthetic/orthotic devices.
• Maintain accurate clinical documentation in patient records.
• Participate in home health team meetings and in-service trainings.
• Perform other job-related duties as assigned.

Provides direct physical therapy according to directions of physical therapistand in accordance with the physician's plan of care.2. Treats patient to relieve pain, develop or restore function, and maintain maximum performance.3. Directs and aids patients in active and passive therapeutic exercises, musclere-education, gait and functional training, transfer activities and prosthetictraining.4. Utilizes physical agents, including, but not be limited to heat, cold, water, light,electricity, ultrasound and massage.5. Observes and reports to the physical therapist and the Organization person-nel the patient’s reaction to treatment and any changes in patient’scondition.6. Instructs patients in care and use of wheelchairs, braces, crutches, canes,and prosthetic or orthotic devices.7. Maintains necessary records; clinical notes and conference notes, which willbe incorporated into the patient’s clinical record in prescribed time frames.8. Participates in Organization Home Care Team Members meetings andOrganization in-service Education Program.9. Possesses and maintains current CPR certification.* The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
