Job summary

Paycom - ATS is seeking a Physical Therapy Assistant in the home health setting in Jacksonville, FL. The role involves delivering PT services in patients' homes under a PT's supervision and coordinating with a multidisciplinary team to address mobility and functional goals.

Responsibilities

• Participate in in-office or telephonic case conferences with the interdisciplinary team.
• Coordinate assessment findings and discharge planning with the HH treatment team.
• Instruct patients and families in care and use of physical therapy devices and plan of care.
• Provide home-based physical therapy services as prescribed by a physician.
• Evaluate patients using diagnostic tests and functional assessments.
• Supervise and mentor PTA as outlined in policy while collaborating with teachers, physicians, psychologists, social workers and other professionals.

Attend required training on HH clinical programs2. Attend all in-services and training related to HH regulatory changes and PT practice related to our current HH polices and procedures.3. To instruct other health care team personnel in certain phases of physical therapy with which they may work with the patient.4. Counsel patients to help them understand their disorder.5. Instruct the patient and family in care and use of physical therapy devices and all aspects included in the patient’s plan of care.6. Work with patients of all ages.7. Provide physical therapy services as prescribed by a physician which can be safely provided in the home.8. Evaluate patients by applying diagnostic muscle, nerve, joint and functional abilities tests (PT only).9. PTA will progress a patient using appropriate treatment plan outlined and established by a PT.10. Work with teachers, physicians, psychologists, social workers, and other helping professionals to address patient mobility and other physical therapy related disorders.11.
