Job summary

Paycom - ATS is seeking a licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) in Florida to provide home health PT services across patient homes. The role includes evaluating mobility, developing treatment plans, and mentoring PTA staff.

You will coordinate with interdisciplinary teams, attend required trainings, and ensure adherence to current HH policies and procedures while supporting patients of all ages. Reliable transportation is required.

Qualifications

• Current licensed PT/PTA in the state of Florida.
• Must have degree from an accredited institution.
• Reliable transportation to and from patient homes.
• Current CPR certificate.

Responsibilities

• Participate in in-office or telephonic case conferences with the interdisciplinary team.
• Coordinate assessment findings and DC planning with HH treatment team.
• Attend required training on HH clinical programs and regulatory changes.
• Instruct other health care personnel in PT-related phases for patient care.
• Counsel patients to help them understand their condition and plan of care.
• Instruct patients and families in care and use of PT devices.
• Provide PT services as prescribed by a physician in the home.
• Evaluate patients using standard PT assessment methods.
• Supervise and mentor PTA as outlined in policy.
• Collaborate with teachers, physicians, psychologists, social workers, and other professionals to address mobility and PT-related disorders.

Responsibilities May Include:Participate as needed with in office or telephonic case conferences with the Interdisciplinary team.Coordinate Assessment findings and DC planning with all members of the HH treatment team.Attend required training on HH clinical programsAttend all in-services and training related to HH regulatory changes and PT practice related to our current HH polices and procedures.To instruct other health care team personnel in certain phases of physical therapy with which they may work with the patient.Counsel patients to help them understand their disorder.Instruct the patient and family in care and use of physical therapy devices and all aspects included in the patient’s plan of care.Work with patients of all ages.Provide physical therapy services as prescribed by a physician which can be safely provided in the home.Evaluate patients by applying diagnostic muscle, nerve, joint and functional abilities tests (PT only).PTA will progress a patient using appropriate treatment plan outlined and established by a PT.Work with teachers, physicians, psychologists, social workers, and other helping professionals to address patient mobility and other physical therapy related disorders.PT will supervise and provide mentorship to the PTA as outlined in the Brooks Home Care Advantage Policy and Procedures.Education and QualificationsCurrent licensed PT/PTA in the state of Florida with a degree from an accredited institutionMust have reliable transportation to and from patient homesCurrent CPR certificate.
