Job summary

Priority Dispatch Corp. is seeking a licensed Physical Therapist or Physical Therapist Assistant for home health in Orange Park, FL. This part-time, day-shift role serves patients in their homes and requires transport to visit patient locations.

You will participate in interdisciplinary teams, coordinate assessments, provide instructional care, and ensure adherence to home health policies. Training aligns with regulatory changes and PT practice. Must have Florida license and CPR certification.

Qualifications

• Licensed PT or PTA in Florida with degree from an accredited institution.
• Must have reliable transportation to and from patient homes.
• Current CPR certificate.

Responsibilities

• Participate as needed with in office or telephonic case conferences with the Interdisciplinary team.
• Coordinate Assessment findings and DC planning with all members of the HH treatment team.
• Attend required training on HH clinical programs
• Attend all in-services and training related to HH regulatory changes and PT practice related to our current HH polices and procedures.
• To instruct other health care team personnel in certain phases of physical therapy with which they may work with the patient.
• Counsel patients to help them understand their disorder.
• Instruct the patient and family in care and use of physical therapy devices and all aspects included in the patient’s plan of care.
• Work with patients of all ages.
• Provide physical therapy services as prescribed by a physician which can be safely provided in the home.
• Evaluate patients by applying diagnostic muscle, nerve, joint and functional abilities tests (PT only).
• PTA will progress a patient using appropriate treatment plan outlined and established by a PT.
• Work with teachers, physicians, psychologists, social workers, and other helping professionals to address patient mobility and other physical therapy related disorders.
• PT will supervise and provide mentorship to the PTA as outlined in the Brooks Home Care Advantage Policy and Procedures.
