Job summary

Crossroads Home Health and Hospice seeks a Physical Therapy Assistant to deliver PT services under the supervision of a licensed physical therapist or rehab supervisor. The role involves direct patient care, implementing the plan of care, guiding therapeutic exercises, and using modalities as directed.

A PTA license/certification and CPR certification are required, with the ability to travel as needed within the service area.

Qualifications

• Graduate of an accredited PTA program with an approved Associate’s degree.
• Current license or certification as required by state law.
• CPR certification maintained.

Responsibilities

• Provides direct physical therapy according to directions of a qualified physical therapist and in accordance with the plan of care.
• Takes care of patients to relieve pain, develop or restore function, and maintain maximum performance.
• Directs and aids patients in active and passive therapeutic exercises, gait and functional training, transfers, and prosthetic training.
• Utilizes physical agents such as heat, cold, water, light, electricity, ultrasound and massage.
• Observes and reports patient responses and changes to the physical therapist and organization staff.
• Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic devices.
• Maintains clinical notes and records in the patient’s ongoing chart.
• Participates in interdisciplinary group and organization inservice education program.
• Participates in quality improvement teams and activities.
• Performs other duties as assigned.
