Job summary

Lifepoint Rehabilitation is seeking a Physical Therapist Assistant to join the Powers Health Rehabilitation Health Center on a PRN schedule in Indiana. The role focuses on implementing treatment plans, monitoring progress, documenting outcomes, and educating patients and families to support overall recovery.

Qualified candidates hold an IN-licensed PTA credential, an Associate's degree, and a Basic Life Support certification.

Qualifications

• Education: Associate's degree required.
• Licenses: Physical Therapy Assistant in the state of IN.
• Certifications: Basic Life Support (BLS).

Responsibilities

• Implements treatment plan developed by supervising therapist using appropriate modalities. Seeks consultation as necessary.
• Monitors patient's responses to the treatment plan. Regularly communicates patient progress and possible goal revisions to the physical therapist.
• Documents services provided and patient response/progress.
• Educates the patient and family/caregiver about patient deficits.
• Assists with patient discharge planning. Provides information regarding appropriate selection/use of adaptive equipment and community support programs.
