Community Rehabilitation Hospital South in Greenwood, IN is seeking a Physical Therapist Assistant (PRN) to support our rehabilitation team. You will implement treatment plans under supervision, monitor patient responses, document progress, and educate patients and families while assisting with discharge planning.

Responsibilities

• Implements treatment plan developed by supervising therapist using appropriate modalities; seeks consultation as necessary.
• Monitors patient's responses to the treatment plan and communicates progress to the physical therapist.
• Documents services provided and patient response/progress.
• Educates patient and family/caregiver about deficits.
• Assists with discharge planning and information on adaptive equipment and community support programs.
