Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Newark, OH. The role supports the Physical Therapist to provide therapy services, conduct screenings, and help plan treatment for residents.

The position is part-time or PRN, requires an Associate's degree, an active PTA license, and commitment to ongoing professional development. CMS-compliant documentation in NetHealth EMR is expected, with opportunities for growth.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct both individual and group treatment as assigned, covering strength, mobility, gait, ROM, and modalities.
• Assist in selecting techniques/media and adapt as indicated by resident’s performance.
• Monitor residents’ response to intervention and modify as indicated.
• Assist in establishing and modifying resident schedules with the PT and team.
• Report to work on time and coordinate schedule to achieve productivity.
• Complete compliant and medically necessary documentation in NetHealth EMR as per CMS expectations.
