Qualifications

• Associate’s Degree in Physical Therapy Assistant from an accredited program.
• Current, unencumbered license to practice as a PTA in the state of work.
• Basic computer skills and ability to navigate service delivery software and EHR software.

Responsibilities

• Return residents to optimal physical function in a SNF setting.
• Instruct patients in maintaining function and home exercise programs.
• Collaborate with therapists and physicians to achieve goals.
• Document progress in the EHR and related software.
• Support discharge planning and ongoing patient education.
