Job summary

Porthaven-Post-Acute seeks a dedicated Physical Therapist Assistant to assist the Physical Therapist in delivering resident care in a Portland-area facility. The PTA will document care, support discharge planning, and educate residents and families on exercises, safety, and home programs.

Qualifications

  • Associate degree from an accredited PTA program.
  • Must hold a Licensed PTA in the state and maintain licensure in good standing.

Responsibilities

  • Treat residents as directed by the Physical Therapist.
  • Complete timely, accurate documentation per facility and regulatory requirements.
  • Maintain a safe treatment environment with infection control and safety protocols.
  • Participate in Resident Care conferences or Rehabilitation meetings as needed.
  • Educate residents and families on goals, home exercise programs, and safety techniques.
  • Assist in training Restorative Aides and support staff as needed.
  • Recommend and facilitate ordering of necessary durable medical equipment to support independence.
  • Report equipment problems and contribute to quality assurance initiatives.
