Collaborative interdisciplinary team

Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist according to established plans of care.
Deliver therapeutic exercise, gait training, balance activities, transfer training, and mobility interventions.
Monitor patient progress and communicate changes to the supervising Physical Therapist.
Complete timely and accurate documentation using the electronic medical record (EMR).
Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies.
Collaborate with Physical Therapists and interdisciplinary team members to provide coordinated, patient-centered care.
Maintain a productive clinical caseload while delivering compassionate, high-quality therapy services.
Act as the main liaison between leadership and the facility.
Oversee staff performance and support professional development.
Manage administrative and operational tasks across therapy services.
Coordinate meetings, training, and events for staff and residents.
Maintain communication with residents, families, and leadership.
Drive referrals for all disciplines
Current State Physical Therapist Assistant (PTA) License
Current CPR/BLS Certification
Valid driver's license, reliable transportation, and current auto insurance
Previous experience in geriatrics, home health, assisted living, or skilled nursing preferred
Previous leadership, supervisory, or Director of Rehabilitation experience preferred, but leadership potential will be considered
Strong communication, organizational, and leadership skills
